Introduction

Vagifem (estradiol hemihydrate) is a prescription medication containing estradiol, which is a type of estrogen.

It is primarily used to treat symptoms of vaginal atrophy due to menopause. Vaginal atrophy can lead to vaginal dryness, irritation, and painful intercourse (dyspareunia).

Estradiol helps replenish estrogen levels in the vaginal tissues, reducing the symptoms associated with estrogen deficiency.

Brand name: Vagifem (generic name: estradiol hemihydrate).

Form: Vagifem is available in the form of vaginal tablets (inserted directly into the vagina).

How to Use

For vaginal atrophy due to menopause: The usual dose is one 10 mcg tablet inserted vaginally once a day for the first 2 weeks, then reduced to one tablet twice a week after that for maintenance therapy.

For postmenopausal women with a uterus, a progestogen (e.g., progesterone) is also commonly recommended to help reduce the risk of endometrial cancer associated with unopposed estrogen therapy.

Administration:

  • Insert the Vagifem tablet deep into the vagina, preferably at bedtime to reduce any discomfort.

  • Use the tablet with clean, dry hands. The kit will include an applicator for easier insertion.

  • Do not use Vagifem if the vaginal area is irritated or infected, unless directed by a healthcare provider.

Timing: For maximum effectiveness, follow your healthcare provider’s instructions on when and how often to use Vagifem.

Missed Dose: If you miss a dose, insert the tablet as soon as you remember. If it's nearly time for the next dose, skip the missed dose—do not double the dose.

Discontinuation: If you need to stop using Vagifem, consult your healthcare provider about the recommended timing.

Common Questions

Can I take Vagifem with other medications?

Vagifem (estradiol hemihydrate) can interact with certain medications, including other hormone therapies, blood thinners, and anticonvulsants. Always inform your healthcare provider about all the medications you're taking, including over-the-counter drugs and supplements.

Is it safe to drink alcohol while using Vagifem?

Moderate alcohol consumption is generally considered safe while using Vagifem (estradiol hemihydrate), but excessive drinking may increase the risk of side effects, especially related to the liver or blood pressure. Always follow your healthcare provider's advice regarding alcohol consumption.

How long does it take for Vagifem to start working?

Vagifem (estradiol hemihydrate) may take several days to a few weeks to start relieving symptoms of vaginal dryness and discomfort. Continued use is recommended in some instances. In general, hormone therapy is recommended for the lowest dose and the shortest duration possible.

Can I use Vagifem if I’m pregnant or breastfeeding?

Vagifem (estradiol hemihydrate) should not be used during pregnancy, as it may harm the fetus. It is also not recommended for breastfeeding, as estradiol may pass into breast milk. Consult your doctor if you're pregnant, planning to become pregnant, or breastfeeding.

Will Vagifem cause weight gain?

Weight gain is not a common side effect of Vagifem (estradiol hemihydrate), but hormonal therapies can sometimes cause water retention or changes in fat distribution. Talk to your healthcare provider if you notice significant or rapid changes in weight.

What should I do if I miss a dose of Vagifem?

If you miss a dose, insert the tablet as soon as you remember unless it's almost time for the next dose. Do not double the dose to make up for a missed dose.

Can Vagifem be used for long-term treatment?

Vagifem (estradiol hemihydrate) can be used for long-term management of vaginal atrophy, but regular follow-ups with your healthcare provider are recommended to monitor the effectiveness and minimize potential risks of prolonged estrogen use, such as an increased risk of breast or uterine cancer. In general, hormone therapy is recommended for the shortest duration possible to help manage the risk of side effects.
